Overview

Responsibilities
- Conduct psychosocial assessments of patients and families to identify needs and plan interventions.
- Develop and implement discharge planning and care coordination with the patient, family, and care team.
- Provide counseling, crisis intervention, and referrals to community resources as needed.
- Document assessments, care plans, progress notes, and discharge summaries in the medical record, maintaining HIPAA compliance.
- Collaborate with physicians, nurses, and other social workers to address barriers to discharge and arrange post-acute services.

Requirements
- Active Georgia LCSW license
- Minimum 2 years of experience in the Medical Social Work specialty; acute hospital experience preferred
- Experience with discharge planning, care coordination, and resource linkage
- Ability to work night shift and fulfill a 12-week travel assignment with guaranteed 40 hours per week
- Must be eligible to work in the United States
