Coordinating care for diverse patient populations, the full-time Care Coordinator - New York Licensed will provide guidance, support, and education to patients while collaborating with care teams to implement comprehensive care plans in a hybrid work environment that includes on-site visits and remote work. Key responsibilities Develop and implement comprehensive care plans in collaboration with the patient's care team Provide health education related to symptom management and preventive care to patients and caregivers Utilize behavioral change modalities to coach patients and families on lifestyle modifications Required qualifications Bachelor's degree Healthcare and/or community-based experience with diverse populations and complex chronic illnesses preferred Proficiency in MS Office and ability to learn hospital documentation and care coordination applications Minimum of one year in a relevant position Bilingual (English/Spanish) preferred
